  • Contact employers and public and private institutions to promote training, self-employment and employment for participants with the purpose of maintaining a resource bank.
  • Document the participant's employment and work promotion efforts in an electronic file. This includes: evaluating the progress of participants in ecological assessments, vocational workshops,
    employment, self-employment and training in the intensive, moderate and follow-up phase.
  • Provide follow-up to training and self-employment projects through visits and calls
    telephone calls to service providers.
  • Attend workshops, training and training activities for your professional development.
  • Organize field visits to factories, companies and industries to stimulate the skills and potential of
    employment of the participants.
  • Prepare and deliver weekly work plan to the Director and clinical coordinator of the Transitional Center
    of Services (CTS).

Service Mediator

  • Guaranteed the quality of the services received by each participant located in SSPADI community homes and the implementation of the Community Based Plan.
  • Mediated participant cases for the rest of the team including social workers, nurses, psychologists, occupational and recreational therapists, and rehabilitation counselors.
  • Collaborated as a member of the Interdisciplinary Team (EID) in the evaluation, planning and implementation of services, for the benefit of the participant. The approach is aimed at educating, planning, referring, coordinating, evaluating, advocating, among others.
  • Acted as the link between the components of the EID, the service provider, support group and other people of the SSPADI.
  • Visited 4 community homes for adults with intellectual disability in a weekly basis. To ensure that each participant is given the support they need and their civil rights and everything related to health, education, rehabilitation, family life, inclusion and integrity in the community, quality of life, among others, are protected.
  • Provide case follow-up for a total of 24 participants.
  • Maintained an empathetic and gentle relationship with each assigned participant. Also, spent quality time with participants in each visit to learn their need and provide better recommendations to the team.
  • Conduct preliminary investigations of incidents that occurred in group and foster homes and ensure that I act in the best interest of the participant and in accordance with the procedures established by the SSPADI Quality Unit.
  • Documented all interventions in the forms established by SSPADI and the electronic file.
  • Evaluated the activities of the homes, ensuring that two external inclusion outings to the community and two internal activities were carried out.
  • Took part in extra-curricular activities with the participants realized in and out of the service center.
  • Participated actively in the process of transitioning and placing a participant into a new community home.
  • Provided General Orientation to every employee in charge of a new hire in the community homes.
  • Participated in training, meetings, conferences and other activities related to the area of work.
  • Attended bimonthly meetings of the Service Mediators convened by the Area Coordinator.
